Clinical Analysis of 180 Cases of Thyroid Diseases


Abstract
The thyroid diseases are the most common in endocrine diseases. Improving the pathology, physiology, diagnostic method and treatment of thyroid disease, the problem of treatment is yet controversial in some cases, especially thyrotoxicosis and malignant disease. The most important problem is against recurrence.

Authors made clinical studies about 180 cases of thyroid diseases during past 4 years and 8 -months from May, 1972, to December, 1976, in the department of Surgery, Han Yang University Hospital and obtained the following results.

1) The thyroid diseases were commonly found in age of 21~50 year (69.9%) especially in 4th decade (27.8%).
2) Sex distribution revealed preponderance of female with ratio of 8: 1.
3) The duration of illness was from below 3 months to over 10 years. The most common number -of duration was 12 month (53. 1%).
4) The most common frequent clinical symptoms and signs were palpation of mass on anterior neck (85.5%), tremor (5.5%), palpitation (45.5%), perspiration(19.5%), nervousness(19.5%), fatigue (19.5%) and anorexia (16.7%).
5) The B.M.R. was relatively significant to serve as a guide for the diagnosis and treatment of thyrotoxicosis. B.M.R. of thyrotoxicosis patients was above +31%.
6) In histopathological analysis of 92 cases, which were operated, benign tumor was 24 cases =(26%), cyst 23 cases (25.4%), goiter 10 cases (10.7%), thyrotoxicosis 25cases(26.5) throiditis <1. 1%) and carcinoma 9 cases (9.7%).
7) In operative technique, of 92 cases, we used subtotal thyroidectomy for 26 cases hemilobectomy for 25 cases, mass extirpation for 25 cases, partial lobectomy for 9 cases, total thyroidectomy plus radicalneck dissection for 4 cases, total thyroidectomy for 1 cases and biopsy for 2 cases.
8) Post-operative complication were hypoparathyroidism 1 cases, temporary hoarsess 2 cases, bleeding lease, and wound infection 3 cases.
